Nonsense-Based Health Care - in the Service of Political Ideology and Sectarian Beliefs
ConclusionSince 2016, we have seen increasing attempts to distort or ignore medical science, clinical and epidemiological research findings to support the political ideology of the ruling party and the religious beliefs of their extreme fundamentalist supporters.  As we have discussed, most recentlyhere, the Trump regime has seen fit to put ill-informed people in positions of power in health care and public health agencies.  Some of these people have put their political and/or religious agendas ahead of the public ' s health.  Our examples above show a continuing inclination by the administration, its sympat...

Specialist pediatric palliative care prescribing practices: A large 5-year retrospective audit
Conclusions: Audit of the prescribing practices in specialist pediatric palliative care service shows that knowledge of pediatric palliative care formulary, rational drug use, dosing, and prescribing guidelines is essential for symptom control in children with advanced life-limiting illness. Noncompliance to WHO prescribing guidelines in one fourth of cases and using nongeneric names in two-thirds of prescription indicates poor prescribing practices and warrants prescriber education. Prescription noncompliance by almost two-thirds of patients is alarming and necessitates intense patient, family, and caregiver education and empowerment.

Airway Microbiota Is Associated with Up-Regulation of the PI3K Pathway in Lung Cancer.
CONCLUSIONS: The data presented here shows that several transcriptomic signatures previously identified as relevant to lung cancer pathogenesis are associated with enrichment of the lower airway microbiota with oral commensals. PMID: 29864375 [PubMed - as supplied by publisher]

Utilization trends and safety of intravenous iron replacement in pediatric specialty care: A large retrospective cohort study
ConclusionsIV iron is commonly prescribed by certain pediatric specialties but there is little standardization in the indications, formulations, or dosing. These data suggest that IV iron should be considered a safe alternative for iron deficiency treatment in pediatrics when oral iron is either unsuccessful or contraindicated.

Low-dose oral ketamine as a procedural analgesia in pediatric cancer patients undergoing bone marrow aspirations at a resource-limited cancer hospital in India
Conclusion: We found no significant effects on the procedural pain in any of the treatment groups compared to placebo. There were only mild side effects. The doses of ketamine might be insufficient for this painful and stressful procedure.

Admission patterns and outcomes of postoperative oral cavity and oropharyngeal cancer patients admitted to critical care in the UK: An analysis of the Intensive Care National Audit and Research Centre Case Mix Programme database
We examined a national intensive care database to assess patterns of admission and outcomes for patients following surgery for malignancies of the oral cavity and oropharynx. An analysis was performed of the Intensive Care National Audit and Research Centre (ICNARC) Case Mix Programme database. Data were extracted on case mix and outcomes for patients coded as ‘malignant neoplasm of the oropharynx requiring surgery’ admitted to critical care between 2010 and 2019.

Voriconazole: How to Use This Antifungal Agent and What to Expect
Semin Respir Crit Care Med 2015; 36: 786-795DOI: 10.1055/s-0035-1562903Voriconazole is an important agent in the antifungal armamentarium. It is the treatment of choice for invasive aspergillosis, other hyaline molds, and many brown-black molds. It is also effective for infections caused by Candida species, including those that are fluconazole resistant, and for infections caused by the endemic mycoses, including those that occur in the central nervous system. It has the advantage of being available in both an intravenous and an oral formulation that is well absorbed. Drawbacks to the use of voriconazole are that it has un...

How Variable Is Our Delivery of Information? Approaches to Patient Education About Oral Chemotherapy in the Pediatric Oncology Clinic
In pediatric patients with acute lymphoblastic leukemia, adherence to oral chemotherapy relies largely on a parent's comprehension of the drug's indication and administration guidelines. We assessed how pediatric oncology providers educate families about oral chemotherapy. We conducted a cross-sectional survey of 68 physicians and nurses from 9 institutions in the Dana-Farber Cancer Institute Acute Lymphoblastic Leukemia Consortium. The inter-individual approach to patient education is variable and may consist of handouts, treatment calendars, and discussions.
